Both sides went into the game level on points in Group H, but the Danes secured the automatic spot for Poland and Ukraine with a hard-fought victory.
The home side got off to a perfect start when Michael Krohn-Delhi fired them in front after just 13 minutes.
Sunderland loanee Nicklas Bendtner doubled Denmark's lead on 63 minutes to put the hosts in firm control.
Portugal pulled a goal back through Cristiano Ronaldo two minutes into stoppage time with a dipping free-kick, but it was not enough as Denmark progressed meaning Portugal will have to content themselves with a place in the play-offs.
Norway
Norway ended their campaign on a high with a 3-1 victory over winless Cyprus.
Morten Gamst Pedersen set Norway on their way with the opener on 25 minutes and West Ham striker John Carew added a second nine minutes later.
Yiannis Okkas gave Cyprus hope when he reduced the arrears three minutes before the break.
However, Tom Hogli made the game safe for Norway with the home side's third goal on 65 minutes.
